Papel de enfermería en los códigos tiempo-dependientes: códigos sepsis

Abstract

Resumen: Introducción: La sepsis es un problema de salud mundial con unas tasas de incidencia que se han visto aumentadas en los últimos años. Como respuesta a este problema se crea en 2004 la Campaña Sobrevivir a la Sepsis con el objetivo de reducir su morbimortalidad. En base a esta campaña y con el mismo objetivo de combatir esta enfermedad tiempo-dependiente se crearon protocolos como el código sepsis. Objetivos: El objetivo principal de este trabajo fue el de describir y analizar la importancia del CS como código tiempo-dependiente y el papel de enfermería en el desarrollo del mismo en base a la evidencia disponible. Los específicos fueron describir qué es el CS y por qué se considera un Código tiempo dependiente así como analizar y describir el papel de enfermería en la detección precoz y el tratamiento de la sepsis. Metodología: Se realizó una revisión bibliográfica entre Febrero y Abril de 2020 sobre la implementación del código sepsis y el papel que desempeña enfermería en el mismo, en el ámbito de los SUH. La búsqueda se realizó en las bases de datos PubMed-Medline, MEDES y Google académico. Resultados: Los resultados de la búsqueda inicial fue de 2902 artículos, reduciendo este número tras aplicar los criterios de inclusión y exclusión como se indica en el flujograma (Ilustración 1). Finalmente se seleccionaron 13 artículos. Discusión: En líneas generales los autores revisador coinciden en importancia que tiene el CS en la detección precoz y reducción de la morbimortalidad del paciente séptico. Incidiendo en el papel clave de enfermería tanto para el cumplimiento del código como para el desarrollo del mismo, desarrollando en el ámbito de urgencias una importante labor desde el triaje en beneficio del paciente. Aun así, se encuentra la necesidad de continuar investigando ya que se cree que existe un gran margen de mejora respecto a la sepsis y shock séptico. In response to this problem, the Surviving Sepsis Campaign was created in 2004 with the aim of reducing its morbidity and mortality. Based on this campaign and with the same objective of combating this time-dependent disease, protocols such as the sepsis code were created. Objetives: The main objective of this work was to describe and analyze the importance of CS as a dependent time code and the role of nursing in its development based on the available evidence. The specifics were described what CS is and why it is considered a time dependent code, as well as analyzing and describing the role of nursing in the prior detection and treatment of sepsis. Methods: A bibliographic review was carried out between February and April 2020 on the implementation of the sepsis code and the role that nursing performs in it, in the ED. The search was performed in the PubMed-Medline, MEDES and Google academic databases. Outcomes: The results of the initial search was 2902 articles, reducing this number after applying the inclusion and exclusion criteria as indicated in the flowchart (Illustration 1). Finally 13 articles were selected. Discussion: In general, the review authors agree on the importance of CS in the early detection and reduction of morbidity and mortality of the septic patient. Emphasizing the key role of nursing both for compliance with the code and for its development, in the emergency area an important task from triage for the benefit of the patient. Even so, there is a need to continue investigating since it is believed that there is a great margin of improvement regarding sepsis and septic shock
