Depresión post-ictus: eficacia de las terapias farmacológicas y no farmacológicas

dc.description.abstract | Objetivo: El objetivo de este trabajo es estudiar la eficacia de las terapias farmacológicas y no farmacológicas en la depresión post-ictus. Diseño y método: El diseño corresponde a una revisión sistemática. Se incluyeron ensayos clínicos de la base de datos de Pubmed y Web of science, con artículos científicos desde 2007 hasta 2022. Resultados: Se incluyeron un total de 10 artículos. En nuestros resultados se observan los beneficios y complicaciones de tratamientos farmacológicos, de neuroestimulación, de intervenciones psicológicas y de otros como la acupuntura o terapias herbarias. Respecto a tratamientos farmacológicos: la fluoxetina reduce la probabilidad de depresión a los 6 meses, pero aumenta las fracturas óseas; el cilostazol se considera un complemento eficaz y seguro; el escitalopram, comparado con la terapia de resolución de problemas, tiene la capacidad de reducir la enfermedad tras 12 meses de tratamiento. Por otro lado, el estimulador eléctrico mastoideo percutáneo junto a antidepresivos reduce la incidencia y mejora la cognición. En cuando a intervenciones psicológicas, el modelo de atención integrada detecta síntomas depresivos y protege contra la depresión post-ictus; la rehabilitación integral influye positivamente en aspectos motores y sensoriales, además de aportar confianza al paciente; el programa de intervención de factores de riesgo multifactorial también aporta beneficios emocionales. Otras terapias como la acupuntura y las herbarias, como adyuvante a las farmacológicas, han sido eficaces ante la depresión post- ictus. Conclusiones: En general, los tratamientos farmacológicos mejoran la depresión post- ictus, pero cuentan con efectos adversos como fracturas óseas; las intervenciones psicológicas generan beneficios de autoestima, además de mejoras motoras o sensoriales reduciendo el gasto sanitario; y terapias como la acupuntura o las herbarias, aún en progreso de estudio como coadyuvante, puede reducir efectos adversos de la terapia farmacológica. ABSTRACT Objective: The objective of this work is to study the efficacy of pharmacological and non-pharmacological therapies in post-stroke depression. Design and method: The design corresponds to a literature review. Clinical trials from the Pubmed and Web of science databases were included, with scientific articles from 2007 to 2022. Results: A total of 10 articles were included. Our results show the benefits and complications of pharmacological treatments, neurostimulation, psychological interventions and others such as acupuncture or herbal therapies. Regarding pharmacological treatments: fluoxetine reduces the probability of depression at 6 months, but increases bone fractures; cilostazol is considered an effective and safe supplement; escitalopram compared with problem-solving therapy has the ability to reduce the disease after 12 months of treatment. On the other hand, the percutaneous electrical mastoid stimulator together with antidepressants reduces the incidence and improves cognition. Regarding psychological interventions, the integrated care model detects depressive symptoms and protects against post-stroke depression; Comprehensive rehabilitation has a positive influence on motor and sensory aspects, in addition to providing confidence to the patient; the multifactorial risk factor intervention program also provides emotional benefits. Other therapies such as acupuncture and herbal complementary to pharmacological have been effective against post-stroke depression Conclusions: In general, pharmacological treatments improve post-stroke depression, but they have adverse effects such as bone fractures; psychological interventions generate self-esteem benefits, in addition to motor or sensory improvements, reducing health spending; and therapies such as acupuncture or herbal therapies still under study, in a complementary way can reduce adverse effects of pharmacological therapy. | es_ES |
