dc.description.abstract | La principal causa de muerte tras sufrir un traumatismo es la hemorragia, siendo esta responsable del 80% de las muertes en las primeras horas. Esta pérdida de sangre incontrolada puede agravarse por la aparición de la Coagulopatía Inducida por el Trauma (CIT), un fallo global en el sistema de coagulación que impide una adecuada hemostasia. Diferentes estudios han evidenciado los beneficios del ácido tranexámico (ATX) en estas situaciones, sin embargo, en la actualidad su administración es limitada. Objetivo: Sintetizar la mejor evidencia científica sobre el uso de ATX en el manejo de la hemorragia traumática aguda. Metodología: Se ha llevado a cabo una revisión bibliográfica narrativa durante un periodo de tres meses comprendido desde noviembre de 2021 a enero de 2022. La búsqueda de información se ha centrado en 3 bases de datos: Pubmed-Medline, Scielo y Cochrane library. Se incluyeron estudios en inglés y español centrados en la administración de ATX tanto en el ámbito extrahospitalario como hospitalario. Resultados: Tras aplicar los criterios de selección se han recopilado un total de 14 artículos. Las variables analizadas fueron mortalidad temprana y tardía, eventos tromboembólicos y complicaciones asociadas al uso de ATX, días de ingreso en planta y UCI, necesidad de transfusión y, dosis e intervalo de tiempo seguro en la administración del fármaco. Conclusión: El ATX debe administrarse tan pronto como sea posible ante un sangrado significativo o la sospecha de este. Aún es escasa la evidencia para extender su uso en el TCE aislado y traumatismo pediátrico. The leading cause of death following trauma is haemorrhage, accounting for 80% of deaths in the first few hours. This uncontrolled blood loss can be aggravated by the appearance of Trauma-Induced Coagulopathy (TIQ), a global failure in the coagulation system that prevents adequate haemostasis. Several studies have shown the benefits of tranexamic acid (ATX) in these situations, however, its administration is currently limited. Objective: To synthesise the best scientific evidence on the use of ATX in the management of acute traumatic haemorrhage. Methodology: A narrative literature review was conducted over a three-month period from November 2021 to January 2022. The search for information focused on 3 databases: Pubmed-Medline, Scielo and Cochrane library. Studies in English and Spanish focusing on ATX administration in both outpatient and inpatient settings were included. Results: After applying the selection criteria, a total of 14 articles were collected. The variables analysed were early and late mortality, thromboembolic events and complications associated with the use of ATX, days of admission to the ward and ICU, need for transfusion, and dose and safe time interval for drug administration. Conclusion: ATX should be administered as soon as possible in the presence of significant bleeding or suspected bleeding. There is still little evidence to extend its use to isolated TBI and paediatric trauma. | es_ES |
