Bronquiolitis aguda: Estadificación de gravedad y comparativa entre escalas

Abstract

Actualmente, la bronquiolitis aguda se considera la infección respiratoria más frecuente entre los lactantes de nuestro medio. Para determinar el grado de afectación de una manera objetiva se emplean diversas escalas, no habiéndose establecido ningún sistema de referencia para dicha estadificación. El objetivo principal de este estudio es comparar la situación clínica del paciente y el lugar de manejo de su patología con la gravedad estimada por las escalas y comprobar cuál de ellas tiene una mayor concordancia con la situación clínica inicial. Dentro de los objetivos específicos se recogió cuál de estas escalas establece mayor concordancia según la edad del paciente y tras 48 horas de ingreso, el nivel de acuerdo entre escalas y las variables que predicen mejor las necesidades de ingreso hospitalario. Para ello se realizó un estudio descriptivo transversal, siendo nuestra población de estudio los lactantes menores de 2 años diagnosticados de bronquiolitis aguda en el Hospital Materno Infantil Torrecárdenas entre octubre de 2021 y febrero de 2022. La población del estudio contó con 377 pacientes. Se recogió la información sobre los pacientes para la estadificación mediante escalas durante el contacto asistencial con los mismos. Tras ello, se realizó un análisis bivariante mediante Gamma de Goodman y Kruskal y multivariante mediante regresión logística binaria tras la aplicación de las escalas. Se determinó que la escala de Tal modificada por McCallum es la que presenta mayor concordancia con la gravedad clínica de los pacientes (0,836; p <0,001), obteniéndose los mismos resultados para pacientes con edades comprendidas entre los 0 y los 10 meses. Además, la escala de Tal es la que predice mejor la necesidad de ingreso hospitalario de los pacientes afectos de bronquiolitis aguda.

Nowadays, acute bronchiolitis is considered the most common respiratory infection among infants in our setting. Several scores are used to determine the degree of involvement objectively, and no reference system has been established for this staging. The main objective of this study is to compare the clinical situation of the patient and the place of management of his pathology with the severity estimated by the scores and to verify which of them has a greater agreement with the initial clinical situation. Within the specific objectives, it was collected which of these scores establishes greater agreement according to the age of the patient and after 48 hours of admission, the level of agreement between scores and the variables that best predict the needs for hospital admission. For this, a cross-sectional descriptive study was carried out, with our study population being infants under 2 years of age diagnosed with acute bronchiolitis at the Hospital Materno Infantil Torrecárdenas between October 2021 and February 2022. The study population included 377 patients. Information used for calculate staging was collected for during care contact. After that, bivariate analysis was performed using Goodman and Kruskal's Gamma and multivariate analysis using binary logistic regression after applying the scores. It was determined that the Tal score modified by McCallum is the one that presents the greatest agreement with the clinical severity of the patients (0.836; p <0.001), obtaining the same results for patients aged between 0 and 10 months. In addition, the Tal score modified by McCallum is the one that best predicts the need for hospital admission in patients with acute bronchiolitis.
