Revisión bibliográfica: eficacia del tratamiento de la vejiga hiperactiva a través de la estimulación del nervio tibial

Abstract

RESUMEN: Introducción: El síndrome de vejiga hiperactiva se caracteriza principalmente por la necesidad de orinar 8 o más veces a lo largo del día. Convencionalmente se trata con fármacos que, por sus efectos colaterales, conllevan que la adherencia de los pacientes no siempre sea la esperada. Objetivo: El objetivo principal de esta revisión bibliográfica es valorar la eficacia del tratamiento a través de la estimulación percutánea del nervio tibial (PTNS) en el síndrome de vejiga hiperactiva. Metodología: Se realizó una revisión sistemática en 4 bases de datos diferentes, empleando los siguientes descriptores: “Percutaneous Tibial Nerve Stimulation”, “electric stimulation therapy”, “overactive urinary bladder”, “electrostimulation” y “tibial nerve” en inglés y “nervio tibial”, “Estimulación Percutánea del Nervio Tibial” y “vejiga hiperactiva” en español. Resultados: Tras la búsqueda bibliográfica se realizó una lectura de los artículos seleccionados y se eligieron aquellos que cumplieron los criterios de inclusión y exclusión, siendo finalmente un total de 5 artículos. Discusión: Gracias a los resultados observados en los diferentes estudios seleccionados se puede saber que el tratamiento con PTNS tiene por lo general efectos positivos en la sintomatología y la calidad de vida. Conclusiones: La terapia con PTNS ha resultado eficaz para el tratamiento en personas con vejiga hiperactiva, con buenos resultados a medio plazo, los cuales cesan pasado un tiempo, por lo que los pacientes tienen que volver a ser tratados cuando la sintomatología empeora.

ABSTRACT: Introduction: Overactive bladder syndrome is mainly characterized by the need of urinate 8 or more times throughout a day. Conventionally, it is treated with drugs that, due to their side effects, makes that patient adherence is not always as expected. Objective: The main objective of this literature review is to assess the efficacy of treatment through percutaneous tibial nerve stimulation (PTNS) in overactive bladder syndrome. Methodology: A systematic review was performed in 4 different databases, using the following descriptors: "Percutaneous stimulation of the tibial nerve", "electrical stimulation therapy", "overactive urinary bladder", "electrostimulation" and "tibial nerve" in English and “nervio tibial”, “Estimulación Percutánea del Nervio Tibial” and “vejiga hiperactiva” in Spanish. Results: After the bibliographic search, a reading of the selected articles was made and those that met the inclusion and exclusion criteria were chosen, finally being a total of 5 articles. Discussion: Thanks to the results observed in the different selected studies, it can be known that treatment with PTNS generally has positive effects on symptoms and quality of life of patients with overactive bladder (OAB). Conclusions: PTNS therapy has an effective result for the treatment in people who suffer OAB, with good results in the medium term, but it decrease over time, so patients have to be re-treated when the symptoms get worse again.
