Intervenciones fisioterapéuticas en el Sistema Oculomotor. Una revisión sistemática.

Abstract

RESUMEN: Introducción: La insuficiencia de convergencia (IC) es un trastorno de la visión binocular, muy común en toda la población. Los tratamientos de esta patología pueden ser diversos y multidisciplinares. Objetivo: Evaluar y comparar las intervenciones fisioterápicas usadas para la patología oculomotora, con el fin de establecer las alternativas más efectivas. Además, conocer terapias no fisioterápicas que pueden resultar eficaces. Material y métodos: Se ha realizado una búsqueda bibliográfica sistematizada en las bases de datos Web of Science (WoS), PubMed, Cochrane, SciELO, Scopus, Dialnet, Lilacs y PEDro, desde el mes de enero hasta abril de 2020. La búsqueda se centró en los estudios publicados en los últimos 10 años. En la evaluación de calidad de los estudios clínicos aleatorizados se aplicó la escala PEDro. Resultados: En la búsqueda en las bases se hallaron 6165 documentos, y 4 estudios fueron provenientes de otras fuentes. Del total, solo 15 reunieron los criterios de elegibilidad. El tratamiento de la IC es multidisciplinar y las intervenciones empleadas engloban técnicas fisioterápicas y no fisioterápicas. Las maniobras fisioterápicas que más se emplean son: terapia manual, técnicas miofasciales, manipulaciones osteopáticas y ejercicios ortópticos. Otra modalidad empleada es la terapia visual, siendo competencia del optometrista. Conclusión: Por un lado, la terapia craneal, la terapia visual y los ejercicios domiciliarios han mostrado ser intervenciones efectivas. Por otro lado, las manipulaciones y técnicas de energía muscular son menos efectivas. No obstante, se considera importante realizar estudios con intervenciones y seguimientos más prolongados, especialmente de terapia manual. Purpose: To evaluate and compare the physiotherapeutic interventions used for oculomotor pathology, in order to establish the most effective alternative. Also, to learn about non-physiotherapy therapies that can be effective. Methods: A systematic bibliographic search was performed on Web of Science (WoS),PubMed, Cochrane, SciELO, Scopus, Dialnet, Lilacs and PEDro databases, it was carried out from January to April 2020. The research was focused on clinical trials and case reports published in the last ten years. PEDro scale was applied in the quality evaluation of the randomized clinical trials. Results: 6165 documents were found on this databases, and 4 studies were from other sources. Of the total, only 15 met the eligibility criteria. The treatment of CI is multidisciplinary and the interventions include physiotherapy and non-physiotherapytechniques. The most used physiotherapy maneuvers are: manual therapy, myofascial techniques, osteopathic manipulations and orthoptic exercises. Another modality used is visual therapy, but these techniques fall under the competence of optometrists. Conclusion: On the one hand, cranial therapy, visual therapy and home exercises have proved to be effective interventions. On the other hand, manipulations and muscle energy techniques are less effective. However, it would be important to conduct studies where both the interventions and the follow-ups are longer, especially in manual therapy.
