Modelo transdiagnóstico para trastornos antisociales en la adolescencia y juventud

Abstract

Los modelos transdiagnósticos han generado importantes avances en la psicopatología actual. El objetivo del estudio fue establecer un modelo transdiagnóstico para los trastornos antisociales en la adolescencia y juventud. En una primera fase se identificaron las variables transdiagnósticas mediante una revisión sistemática. En una selección final de 19 artículos, se obtuvieron evidencias de variables transdiagnósticas enmarcadas en un factor general de psicopatía o externalizante, que integra variables estructurales como los sistemas de inhibición y activación conductual (BIS - BAS) y el afecto negativo como rasgo (NA-rasgo). Estas variables están asociadas a otras explicativas a nivel emocional (des-regulación emocional y evitación experiencial) y cognoscitivo (rumiación-ira). En la siguiente fase, se obtuvieron evidencias de la calidad psicométrica de los instrumentos para medir sensibilidad al refuerzo (BIS BAS IPIP-R), NA-rasgo (PANAS), y rumiación-ira (ARS). Se confirmaron las estructuras factoriales teóricas y se comprobó su invarianza en muestras agrupadas por sexo y edades. Para las medidas psicopatológicas, se usó el cuestionario de agresión reactiva y proactiva (RPQ), que resultó en dos factores oblicuos, pero no invariante entre los grupos y la Escala de Desviación Normativa, versión reducida en español, (NDS). En la última fase de la investigación, se verificó mediante análisis estadísticos multivariados un modelo transdiagnóstico propuesto con una mayor capacidad explicativa para la agresión reactiva (R 2=.35) y proactiva (R2=.28), y menor para los comportamientos antisociales (R2=.17). Las correlaciones entre las variables sugieren una interacción entre la inhibición conductual (BIS) y el NA-rasgo en el incremento de la des-regulación emocional y la agresión reactiva, mientras que la activación conductual (BAS) se asoció con un aumento en la rumiación-ira, las conductas agresivas proactivas y antisociales. En los efectos indirectos se sugirió un mecanismo de mediación entre las variables estructurales BIS, BAS, NA-rasgo, rumiación-ira y los comportamientos agresivos y antisociales, mientras que la des-regulación emocional como mediadora no fue significativa. En conclusión, se generó nueva evidencia de un modelo transdiagnóstico que permitirá avances en la investigación de las psicopatologías antisociales, así como futuras mejoras en la psicoterapia actual. Abstract: Transdiagnostic models have generated advances in current psychopathology. The objective of the study was to establish a transdiagnostic model for antisocial disorders in adolescence and youth. We first identified the transdiagnostic variables through a systematic review. From a final selection of 19 articles, we obtained evidence of transdiagnostic variables framed in a general factor of psychopathy or externalizing, which integrates structural variables such as behavioral inhibition and activation (BIS and BAS), and negative affect as a trait (NA-trait). These variables are associated with other explanatory variables at the emotional level (emotional dysregulation, experiential avoidance), and cognitive level (anger rumination). In the following phase, evidence was obtained on the psychometric quality of instruments to measure sensitivity to reinforcement (BIS BAS IPIP-R), NA-trait (PANAS), and anger rumination (ARS). In this study, theoretical factor structures were confirmed, and their invariance was checked in samples grouped by sex and age. For psychopathological measures, the Reactive and Proactive Aggression Questionnaire (RPQ) was used, which resulted in two oblique factors, but not invariant between the groups; also, the Reduced Normative Deviance Scale (NDS). In the last phase of the investigation, we verified the proposed transdiagnostic model through multivariate statistical analysis that it had a greater explanatory capacity for reactive (R2=.35) and proactive aggression (R2=.28), and a lower capacity for antisocial behavior (R2=.17). Correlations between variables suggest an interaction between behavioral inhibition and NA-trait, in increasing emotional dysregulation and reactive aggression; while behavioral activation, was associated with an increase in rumination-anger, proactive and antisocial aggressive behaviors. On indirect effects, a mediation mechanism between structural variables BIS, BAS, NA-trait, anger-rumination, and aggressive and antisocial behaviors was suggested, while emotional dysregulation as a mediator was not significant. In conclusion, new evidence was generated of a transdiagnostic model that will allow advances in the research of antisocial psychopathologies, contributes to the RDoC model, and future improvements in current psychotherapy.
